Here’s what you need to get your medical marijuana card in New York.
Valid New York ID or Driver’s License
Valid United States passport with proof of New York residency
Valid out-of-state ID with proof of New York residency (car registration, rental agreement, or voter registration)
Current New York government-issued ID
Current resident card for New York (green card)
New York residency proof with Military ID
Temporary DMV form with expired New York ID or photo ID
Out of State ID with Proof of Residency in NY (i.e., rental lease, utility bill, car registration or voter registration)

Patients may use any of the documents listed above as identification for an online Medical Marijuana Card in New York. The United States government must issue any additional forms of identification before you visit a cannabis doctor in person or online.

Unfortunately, birth certificates are not currently accepted at any MMJ Doctors locations.
Medical marijuana patients and recommendations are restricted to New York residents only. You must first establish state residency before you are allowed to become medical marijuana patients in the state of New York.
HOW TO BECOME A PATIENT FOR MEDICAL MARIJUANA IN NEW YORK
Only New York residents, with valid New York identification or proof of residency are allowed to be patients and receive a medical marijuana recommendation and cannabis card. If none of the documents listed above are available, a person may present proof of residency along with a valid New York ID, an out-of-state ID, passport, or another photo ID.
We advise all patients to obtain copies of medical records or other medical documentation from a primary care physician that details a diagnosis prior to your appointment with an online marijuana doctor. Please bring those documents with you to your appointment for a medical marijuana evaluation.
